When the ISO[1] to be booted with refind is made UEFI isohybrid there's a problem with any configuration I came up with so far if the kernel is specified as /syslinux/alt0/vmlinuz: - if the image resides on a CD/DVD then it boots just fine, but - if the image is booted off USB Flash drive then the kernel won't be found; If it is specified as /EFI/BOOT/vmlinuz but isn't copied over to the El Torito FAT image then at least one of the cases wouldn't work either. The currently deployed workaround[2] is to copy the kernel and initrd but that's as bloated as ELILO would require, which is unfortunate since rEFInd boasts driver support and should be able to read the files directly. Maybe it's a local misconfiguration though but *some* of the icons (which are stored in the same directory) would be missing in a different manner with these boot methods which might be related. The effect is stable on two different hardware platforms and virtualbox. So far I was advised to copy the kernel and initrd; the icons issue looks like implementation bugs (and didn't manifest with copy_kernel() being done in mki-copy-efiboot upon further testing on hardware).
